Parents/Guardians of current students must update beginning of the year forms through Final Forms. 

NEW STUDENTS

Parents/Legal Guardians, follow the steps below to create a Final Forms account and complete your student's forms. 

1. Go to https://trotwoodmadison-oh.finalforms.com

2. Locate the parent icon and click NEW ACCOUNT

3. Type YOUR NAME, DATE OF BIRTH, AND EMAIL ADDRESS.  

4. Click REGISTER

5. Check your email for an ACCOUNT CONFIRMATION EMAIL from Final Forms Mailman. Once received and opened, click CONFIRM YOUR ACCOUNT in the email text.

6. CONFIRM YOUR ACCOUNT in the email text.

7. Create your new Final Forms password. 

8. Next click CONFIRM ACCOUNT.

9. Click REGISTER STUDENT for your first child. 

NOTE: You will receive an email within 2 minutes prompting you to confirm and complete your registration. If you do not receive an email, then check your spam folder. If you still cannot locate the Final Forms email, then email [email protected].
